This Terms of service constitutes a legally binding agreement made between you and Employer Direct Healthcare, LLC (“EDHC”) and its affiliated company(ies). Concerning your access to and use of this website, this site is intended to provide assistance with finding a board-certified surgeon. EDHC’s services include assistance in choosing a surgeon for your needs and guiding you towards tailored care with a personalized experience.

Supplemental terms and condition or documents that may be posted on this website from time to time, are hereby expressly incorporated into this agreement by reference.

The information provided on this website is not intended for distribution to or use by any person or entity in any jurisdiction or country where such distribution or use would be contrary to the law or regulation or which would subject EDHC to any registration requirement within such jurisdiction or country. Accordingly, these persons who choose to access the website from other locations do so on their own initiative and are solely responsible for compliance with local laws, if, and to the extent, local laws are applicable.

All users who are minors in the jurisdiction in which they reside (generally under the age of 18) are not permitted to register through this website and/or use EDHC service(s).

You authorize EDHC to furnish electronic Explanation of Benefits (“EOB”) statements related to your use of the services, in lieu of a hard copy EOB statement

You will maintain accuracy of registered information submitted by you.
You will maintain confidentiality regarding your authentication information.
You are not a minor in the jurisdiction in which you reside. Note: If you are a minor, you have received parental permissions to use this website.
Your use of this website and accompanying service(s) does not violate any applicable law or regulation.
You will provide sufficient information to indicate that you are eligible to use this site and are not an automated device or technology.
You will not transfer your account to any other person or entity.
You will not engage in advertising or solicitation to other members, to include junk email (or spam), buy or sell any products or services on this site.
